Heather M. Smith - Condon, 44, of Streator passed away Sunday, December 22, 2019 at O.S.F. St. Francis Medical Center in Peoria.

Mass of Christian Burial will be at 11:30 A.M. Monday, December 30, 2019 at St. Michael the Archangel Church, Streator. Rev. Fr. Simon Taabu will officiate.

Visitation will be from 9-11 A.M. Monday, December 30, 2019 at the Solon-Telford Funeral Home, Streator.

Burial will be in Calvary Cemetery in Ottawa. Cremation rites have been accorded.

Born August 28, 1975 in Streator she was the daughter of George and Sheila (Brown) Smith.

She is survived by a son, Saul Moreno of Streator; daughters, Eden Bruhns of Streator and Maura Condon of Ottawa; her father, George Smith Jr. of Streator; a sister, Jennifer Evans of Peoria; a brother, Patrick (Karen) Smith of Streator; maternal grandfather, James Brown of Grand Ridge; aunts, Karen (Ahmed) Saleh of Streator, Christine (Butch) Benckendorf of Streator; uncles, Kevin (Pat) Brown of Ottawa, James (Cindy) Brown of Palmdale, California and Daniel (Jennifer) Smith of Chaimpaign; she is also survived by many loving nieces, nephews and cousins.

She is preceded in death by her mom, Sheila Rawlins, paternal grandparents, George and Marguerite Smith; and her maternal grandmother, Dolores Brown.

Born and raised in Streator she attended St. Anthony Grade School and Streator High School. She graduated from Ottawa High School. She attended Illinois Valley Community College and earned an Associate’s Degree in Nursing. She was a Registered Nurse.
